IMMUNIZATION REQUIREMENTS
Are your children ready for school? The following are the immunization requirements for students entering school in August at Crete Public Schools.
Kindergarten, 7th Grade & New Students From Out-of-State:
- 3 doses of Dtp-One must be after their 4th birthday
- Td booster required for 7th grade
- 3 doses of Polio
- 2 doses of MMR
- 3 doses of Hepatitis B
- 1 dose Varicella vaccine (2 doses after age 13) or proof of chickenpox disease
- Current physical dated on or after 2/01/08
New Students From Nebraska:
- 3 doses of Dtp-One must be after their 4th birthday
- 3 doses of Polio
- 2 doses of MMR-all children, K-12 must have 2 MMR vaccines to be enrolled.
- 3 doses of Hepatitis B
- 1 dose Varicella vaccine (2 doses after age 13) or proof of chickenpox disease
- No physical is required, but it is recommended.
